Intake Call
Our Patient Care Coordinator will ask you some questions during the intake call.
- Your name
- Child’s name and date of birth
- Cell phone
- Email address
- Home address
- Therapy being requested – Physical, Occupational, Speech, or Feeding Therapy
- Diagnosis or reason your child needs therapy
- Insurance company’s name and phone number
- Subscriber ID and group number
- Subscriber’s full name and date of birth
- Pediatrician or referring provider
- What days and times will work best for your child?
- What days and times will not work for your child and family?
- How did you hear about Agape Pediatric Therapy?
Orders
We will need signed orders for therapy from your child’s primary care provider prior to the initial evaluation.
- We will request orders from your child’s primary care provider.
- We can accept orders from an MD, DO, ARNP, We cannot accept orders from a chiropractor or dentist.
- We advise parents to also request orders for their child to receive therapy at Agape Pediatric Therapy.
- Orders may be faxed to 509-388-2995.
New Patient Paperwork
After your intake call, we will email you a “Welcome Letter”, which will give you access to our Patient Dashboard.
- In the Patient Dashboard, parents will create a user name and password
- Sign patient forms
- Complete your child’s medical history
- After completing forms and medical history, call our office to schedule your child’s initial evaluation.
- ***New Patient Forms and Medical History must be completed online before we can schedule your child’s initial evaluation.***
Initial Evaluation
- Evaluations are typically scheduled for 50-60 minutes.
- We allow only parent(s) and child in the clinic, no siblings at this time. We
We will make exceptions for families with a new baby.
